Part D Patient Transfers
Again for the 2012 plan year, Medicare Part D sponsors who wish to request permission from a beneficiary to fill his/her prescriptions at a different network pharmacy must send a written transfer request to the beneficiary. The beneficiary may provide permission by calling the plan or pharmacy, or via written statement sent in the mail. . Sponsors and pharmacies seeking the transfer are prohibited from making outbound phone calls to the beneficiary seeking the transfer. The Centers for Medicare & Medicaid Services has provided a model transfer letter for sponsors or pharmacies seeking a transfer.
